Hospital-to-Home Transition Kit

An organization kit for families managing the move home after a hospital stay — one place for discharge information, medications as provided, appointments, pending tests, equipment and home services, during a busy and unfamiliar stretch.

  • One place for the information a family is given at discharge
  • Medications, follow-up appointments and pending tests, as you are told them
  • Equipment, supplies and home services, kept together
  • A Transition Summary that fills in automatically from the workbook

The Hospital-to-Home Transition Kit will be sold on Etsy.

Who it is for

Families bringing a parent or relative home after a hospital stay, and whoever is coordinating the first weeks afterward — whether that is one person or a rotation of siblings.

It is designed for a move home. It is not designed for a move to a nursing facility, a rehabilitation center or assisted living.

What it helps with

Discharge tends to happen quickly, with a lot of information handed to whichever family member happens to be there. The questions come later, at home. The kit is a single place to write down what you were told, so the answers are on paper rather than spread across memories, texts and the back of an envelope.

What is included

  • Workbook (XLSX, Excel 2019 or later) — one place to organize the move home, with a Transition Summary that fills in automatically from what you enter.
  • Printable transition packet — a US Letter PDF to print and write on.
  • Start Here guide — a PDF on setting up and using the kit.

Together they cover what to settle before leaving the hospital, a discharge snapshot, home instructions, medications as provided, follow-up appointments, pending tests and results, warning signs and who to call as the care team describes them, equipment and supplies, home services and support, questions and follow-ups, documents, and the first days at home.

How it is used

Start it at discharge, or as soon as you are home. Keep it somewhere everyone helping can find it, and add to it as appointments are booked and questions are answered. When someone new arrives to help, it is the thing to hand them first.
